Item shows signs of wear/use with bending, creasing and tearing. Outside cover shows heavy dirt marks on front and back. Back cover has missing corner piece.  No writing identified on inside. Pages are torn on bottom edge along spine of booklet.  Booklet/report has been folded in the middle.  May have discoloration due to age/storage.  See photos for visual.
